Three people died and one was injured in an accident during lifeboat maintenance work on a Floating Storage and Offloading (FSO) vessel, which is operated by the Malaysian state energy company Petronas.
Petronas, or Petroliam Nasional Berhad, stated in a communiqué that the incident occurred around 12:50 p.m. (0450 GMT) on Sunday, May 24, on the FSO Sepat, off the state of Terengganu, on the east coast of Peninsular Malaysia.
Authorities confirmed that three of the workers involved in the accident died instantly, while the state company reported that the employees were declared dead upon arrival at the hospital.
The injured operator was urgently evacuated to receive medical treatment and remains currently under observation.
Following the incident, the energy firm indicated that corresponding investigations have already begun to clarify the facts. "Investigations into the cause of the incident are ongoing, in close coordination with the relevant authorities," Petronas detailed, without offering more specific details about the technical failure.
For its part, the Malaysia Marine Department (MMD) reported that "the incident occurred while four personnel were conducting a load test on the lifeboat as part of the required five-year periodic maintenance and inspection, in accordance with the stipulations of Resolution MSC.402(96) of the International Maritime Organization (IMO)."
"Immediately after the incident, the FSO Sepat Emergency Response Team activated response measures, with the support of the assistance vessel MV Kazo Cekap to carry out rescue operations at the scene. Simultaneously, a Medical Evacuation operation involving two helicopters was deployed to transport the victims to Sultanah Nur Zahirah Hospital in Kuala Terengganu for medical treatment," it added.
"The incident site has been secured and access has been restricted for investigation purposes and subsequent technical evaluation. The investigation will examine all relevant aspects to determine the cause of the incident and ensure compliance with prescribed safety procedures," the MMD specified.
